Clinical Care, Delivered Where You Feel Most at Home
El Rapha Cares provides qualified community nursing services so participants can receive skilled clinical care outside of hospital settings. Our registered and enrolled nurses work collaboratively with families, GPs, and allied health professionals to deliver person-centred care that supports recovery, prevents complications, and keeps you comfortable and confident at home.
What we support
Wound & Skin Care
Assessment and management of acute and chronic wounds, pressure injuries, dressings, and skin integrity monitoring to promote healing and comfort.
Medication Management
Safe administration and monitoring of medications, including injections and infusions, along with education for participants and carers.
Health Assessment
Ongoing health monitoring, continence management, care planning, and clear reporting back to your treating clinicians.
