Whether your experience has been positive, challenging, or somewhere in between, we welcome your thoughts. Your input helps us identify what we are doing well and where we can enhance the quality, safety, and consistency of our care.

All feedback is handled confidentially and respectfully. Once submitted, our team will review and respond as needed to ensure your voice is heard and valued.

How we look after it

You can leave your name and email blank and tell us anonymously. We can still act on what you say — we just won't be able to reply.

We won't share what you send with anyone outside El Rapha Cares unless you ask us to, or the law requires it. If we don't end up working together, we delete it.

You can ask us what we hold, have it corrected, or have it deleted, any time — just email admin@elraphacares.com.au. Our Privacy Policy has the full picture, including how to make a complaint.

And you never have to use this form — you're very welcome to just give us a call instead.

You can raise a concern with an independent body at any time. You do not need to tell us first, you do not need our permission, and you do not have to wait for our answer.

Things worth knowing

  • You can raise a concern without giving your name. Our feedback form does not require a name or an email address — we simply will not be able to write back to you.
  • Raising a concern will never affect the care you receive, or how we treat you.
  • Someone else can do it for you. A family member, friend or advocate is welcome to raise a concern on your behalf.
  • If English is not your first language, ring the Translating and Interpreting Service on 131 450 and ask them to connect you to any of the numbers above, or to us.
